To submit a claim for damaged FitnessMats please complete the form below. It is important to include photo images of all damaged mats when submitting a claim.

As promised, we will gladly stand by our FItnessMats and work with you to ensure you are taken care of.

Effective the end of the 1st Quarter of 2024, any FitnessMats with tears by the Grommet holes will no longer be covered under warranty.

All FitnessMats purchased with grommets after September 2022, included Perma-Grommets which are designed to hang and protect the FitnessMats while storing more securely. It is recommended that any FitnessMats purchased prior to September 2022, have the grommets replaced to the Perma-Grommets.
